rubber nipple fell inside the manifold
worst case, it keeps a valve from closing fully and it smacks a piston and you rebuild the engine, best case, it makes it past the intake and exhaust valves and into the cat, melts and possibly damages your cat. take off the intake manifold, you only have to replace a 5dollar gasket to do it.
Don't risk leaving it in there, better safe then VERY sorry, take the tb off and see if you can reach it with long needle nose pliers. If not, take the mani off and get it out.
Well before you do all that try through the throttle body first, it's much easier, but if that doesn't work, then take off the whole mani.
Have you wedged the throttle open with a piece of dowel at the pedal and peeked inside the manifold with a mirror?
I wouldn't start the car yet. There's a possibility the nipple may end up holding one of the valves open, which would cause it to collide with the piston. And that's very bad indeed.

Taking the manifold off is a pain in the ***. I'd remove the throttle body like everyone else said and just try to reach for it. You could also set the pistons so the intake valves are open (exhaust closed) and push compressed air into it. That might shoot her out
