Default Ls tranny no 3rd 5th

My tranny has no 3rd 5th and hard to shift into rev. What would cause this if 1st 2nd and 4th work well would it be an issue with the clutch. Clutch is new sprung 6 puck . I did search but came up empty for B trannys. Also installed password jdm shift linkage bushing kit. I bought this tranny used and put this all in at same time .

Blown transmission, or maybe just a bent shift fork, either way you will need to drop the tranny and open it to find out. 94
