Default 00 Ex Coupe, car is idling very low after warmed up. ~200rpm

Hello, I have a 2000 Honda Civic Ex Coupe automatic with the following modifications...

DC Sports Headers, 4-2-1 Two-piece
AEM Short Ram Intake
Unorthodox Pulleys
Factory exhaust

Recently had the timing belt changed, car has roughly 170k miles on it.

The problem I am having is that after the car has warmed up and I pull up to a stoplight it will idle extremely low around ~200-500rpm. Sometimes when making turns from a stop it will die out, or just die out when I am accelerating from a stop. Also when driving it seems to loose power when I let off the gas as if I would be driving a manual and just down shifted. I'm guessing that it is just running extremely lean and I need to adjust the throttle or maybe the catalytic converter could possibly be clogged up. This car has the original exhaust and it has never been changed out or maintanced, put the aftermarket parts on it within 1yr of buying the car in 2000. So has been running this setup for roughly ~9years. I have been rear-ended twice because of the car dying out at a stoplight in traffic.
