It just started happening about a month ago and I dont know how to get it to turn on normal.
Sometimes, it has trouble starting; ill have to hold the starter, or it will start perfectly, idle at 2000, then die out.
If I hold the gas it sputters, and doesnt rev, then when it does, it goes up after 4000rpm.
I have to let the car warm up for about 3 minutes before it stops doing this. After the car is warmed up the problem goes away.
Funny thing is, my old car (1993 MBZ 190E 2.6) had the exact same problem. Like down to the tee lol, heres a vid. Keep in mind, the car was slightly warmed up prior to this start up.

Mine does the same thing and I know it is the IACV on my car. I swapped down to obd1 from obd2 to run a chipped ecu and have been to lazy to rewire it so therefor its basically not hooked up. I would start with cleaning it like phaphon said above, or pull one from a junk yard and swap it.

I put a B18 throttle body on to replace my stock Y8 and it started idling funky. Could I switch FITV's from the old one?

try cleaning out the one you have

fitv should just make it idle higher though, i don't think it would have an effect on how the car is running like you describe. never know though
