No 4th or 5th gear with hydro B16 tranny

No 4th or 5th gear with hydro B16 tranny
So, I'm putting together a B16A2 swap for my EG. My friend donated to me his old tranny, but he said when it was in his car it wouldn't go into 4th or 5th. I'm just wondering what could cause this as far as something simple or something major, and how much I should be expecting to pay at a shop. Thanks.
