Default OBX Intake Manifold - IACV

I have a 94 ls with a b18b motor fully built and boosted. Its tuned to 14lbs putting out 333hp. I have a OBX stainless IM and i got it tuned without the IACV hooked up. The car stalls when slowing and can not hold an idle. I need to hook up the IACV but there is no where on the IM to do so. Any input as to "rig" or permantly fix this issue. It is not my daily driver but my sunny day cruise car. Please help me.

Well it just snowed here and it is now under a car cover with 2ft of snow on it. I'll try to get it dug out here soon.

Well in that pic the two holes on the back side on the right is where the IAC goes. 2 holes in the middle with a bolt on each side...
