Clutch problem?

I'm not too sure anyone can help with this one.. I have a 95 Accord and have recently noticed a squeak coming from the clutch. The only reason I know it's the clutch is because when I rest my foot on the peddle the squeaking stops. Now this only happens after the engine is warm. I'm also having a problem getting into 3rd and 5th if I run up to 4500rpm and try to go into those particular gears. It's chatters a little, and sometimes a lot... Also the clutch is less than 2 months old..

The clutch was just done before I bought it.. You can tell it's new buy the way it grabs.. I'm going to obviously have to take it back to where it was done..
I want to say its a Pilot Bearing but Accords don't have them like Civics/Tegs do. So I would think its your Throwout Bearing; since, it would press on the diaphragm springs also called fingers which is on the pressure plate.

Update..
The noise is a squeaky clutch arm LOL.. so that's good.. But still can't figure out why it chatters into 3rd and 5th when I'm trying to accelerate at a high rate of speed...
