D series tranny problem

D series tranny problem
The fluid is full and fresh of Honda MTF, 142K miles. 50% of the time it shifts perfectly, no issues at all. The other 50% of the time it either grinds going into 3rd OR will not fully engage in 4th, always one or the other but never both in the same upshift sequence. Any ideas?
