Default B18c crank pulley broke for the second time

Alright guys. This is the second time this has happened and not sure what the hell is causing the crank pulley bolt to break inside the crank. I bought my 96 Integra GSR from a co-worker when I was working at Honda. The crank pulley bolt was broken when he purchased the car. 100% stock motor. He then had a machine shop, re-tap the crank for a larger bolt. Drove it for 10K miles no problem. I get it serviced for a new alternator belt and power steering belt and after a few short miles, I hear a fluttering noise and then a loose bolt bounces under the hood as I'm driving. Yep, crank pulley bolt broke in half, the other still in the crank. I did not even mess with the crank pulley bolt when the belts were replaced. I can't imagine the extra tension from the belts would cause the crank pulley and bolt to fail like that. Got it re-tapped again with the next larger bolt and torqued to spec by hand. About 100 miles later, same damn thing. This crank is cursed. What gives? I've decided to just get a replacement crank either a new OEM B18c crank from Acura or a re-manufactured/aftermarket crank. Please give me some clue of what could be causing the bolt to keep on breaking. The motor had been rebuilt professionally as well with Eagle rods and CP pistons and was turboed last summer with no problems. Ever since the PS and alternator belt was replaced it has been a problem. Could be coincidence or the machine shop just did a poor job re-tapping the threads. The bolt they used looks low grade too. I just want to make sure the same thing does not happen when I get a new crank put in there.

P.S. Now looking for a used B18c crank or recommendations of where to get a re-manufactured or aftermarket crank (Eagle?). Any suggestions on alternatives.
