Default jumping ilde car shakes under load

My idle will jump when I am stopped and the car will almost die then catch it self then almost die until it finally starts to idle normal again. My car is very hard to start cold I have to give it gas until the rpm will stay up to keep the car from dying. When I start my car I notice that there is a smoke coming from my breather filter on my catch can, down in that area. The motor is a usdm gsr motor, and I have cleaned the iavc. My car also shakes under load it is strange because when the car is cold it does not shake it seems to drive fine, but once it heats up it starts to shake it feels like it is in the wheels like axles, ect but I am thinking it is something with the afr messing everything up. ANY help would be much appreciated I am trying to get this thing running right.

is it throwin any codes cause it sounds like we're having a similar problem for me it was the map sensor maybe you need one to.
