When did it last pass smog check?
What's changed since then?
When did you last tune it up?
Any codes?

For one thing, see Honda TSBs 93-009 and 92-041 (concerning c. 1990 Civics that fail state emissions testing). You can get them from http://search.ebscohost.com/. Your library should give you free access to ebscohost.

I am battling to get my 91 Civic to pass now. Start adding Chevron Techron (1 bottle per tank of gas). Search the archives for many more tips, including tuneup, oil change, retarding the timing, looking for an exhaust leak at the downpipe gasket right after the O2 sensor, and using other additives (that are riskier AFAIC than Chevron Techron).
