Its a common thing mostly with aftermarket clutches, just go to the junk yard and get one or if you can weld or know someone that can put some fat beads on it and make it stronger
Let me guess you are not an SI?
CRX Toad has the pictures and we've chatted about this as I've had this problem myself. Non SIs oddly had a different pedal set, and it was weaker.
I've re welded mine and it's stronger then sin.

theres 4 bolts holding the whole assembly on near all four corners of the plate, towards the firewall....disconnect the clutch cable, then gas pedal etc, then go from there....if im not mistaking, bump

The same thing happened to my CR-X. Mines an Si too! Heres a picture of what happened. Some advice? Take off the dashboard! Some people might say that its too much work to take it off, But its NOT! It's more work trying to unscrew the bolts with the dash on! Check out my picture, Maybe this will clarify what your talking about...

Same exact thing happened to me. Just take the steering column out for easier access and i believe there is around 6-8 nuts holding the pedal assembly on. Disconnect clutch/brake/gas and it pulls right out. Then reverse process.
Im surprised this is such a common occurrence. If i would have known that i would have done a write up on it.
