Default I need advice on a tranny issue

Ok, before you think that it is my tranny again lol, hear me out. A friend of mine had a gear selection problem, only to take down his tranny to see one of his shift assemble bolts out and wedged against the assemble, and the other twoo bolts loose . Luckly there was no apparent damage and we tightened them back up and put everthing together. The problem is that now when he goes in reverse the wheels would lockup . He has to tap the gas while the clutch is slightly engaged, for the car to reverse. Anytime he takes his foot all off the clutch it locks up again. Please give me some insight of what is wrong.

I wasn't refering to the bolts that came loose in the tranny, because this was a virgin tranny that had never been opened before yesterday. I am refering to the locking up of the wheels when the car is put in reverse.

when we took the gearbox down tonight, I noticed that he did not have on the washer on his reverse idle gear bolt. It was screwed all the way in without the washer. Could this be his problem? Everything else in the tranny looks good according to the manual.
