Default Fuel cutoff changes randomly -- D16Z6

I have a 95 Civic EX coupe. It has 235k but the previous owner told me the engine was rebuilt and it has around 70k. It runs very strong all the time--never had any starting problems, it idles smooth as butter, and I get good mileage (32-36mpg) BUT I have a weird fuel cutoff issue.

The redline is 7200rpm and I'm assuming the cutoff should be right after that. However, this rarely happens. I've narrowed it down to three scenarios that happen often. BTW, this is in 1st or 2nd gear only, WOT--usually when I'm getting on to an onramp or something.

1. I can feel VTEC come on at 5k, and it pulls from 5000 to 7200 just fine with a fuel cutoff at around 7400.
2. I can feel VTEC come on at 5k, and it pulls until 6600 where I hit fuel cutoff.
3. It feels like VTEC doesn't come on at all and I get a redline at 6600.

Here's a weird twist: when I test drove the car before I bought it, I hit redline a couple times and it was fine. I filled it up with 87 for two tanks and it ALWAYS redlined at 6600 (scenario 3). The first time I filled it up with 89, it hit 7200 and I was amazed--it felt like it had a lot more power. I've been running 89 ever since, and I'd say 9 times out of 10 or more I get scenarios 1 and 2. I haven't yet gone back to 87 btw.

I reset the ECU (pulled out hazard fuse for a few mins) and nothing changed. I took out the ECU and it didn't say what it was on the outer case--I'm assuming (hoping) it's a stock P28.

i have the same motor in my hatch and it would do something kinda like it.. i would be fine have vtec and perfect redline then one day i started it had a cel and it would only go to like 4k after changing out the distributor which was what my code was for the problem still happened and it turned out my ecu was bad.. hope its not hte case but are you getting a cel maybe for ckp thats what mine was but the only solution was to replace the ecu.. i now have a dx p06 because i cant find a virgin p28

That's the thing, I've never gotten a CEL yet in this car. And the light works, it blinks when I turn the key on.

See man, the more we look at this issue, it seems that you have a faulty ECU. It was the first thing I said could be the culprit.
