That is for ground switched H4 harness....Not for 2000 Civic because it is positive switched....for civics 2001-2005 that is right harness...that's what I used

it depends on the ballast...
if the ballast's max draw is 15 amps, then 1 30 amp relay is fine... get a relay that can handle the total amount of amps that both ballast draw...
also, if you have a h4 bulb..
you might want to do this instead:

these diagrams aren't the best... and are sort of confusing... but if you can follow the colors and the line widths... i guess it works... sorta.. https://honda-tech.com/zerothread/1871411
