Mine went at 24,500 miles. I had just put Azenis on and tried to do a Quarter mile run at http://www.racewaypark.com and found out the hard way that the tires really stick Good at 26 psi.
What is your main question anyway?

You can also check the springs on the clutch. If they are loose, it is time to get a new one anyways. Also if you see lots of debris in the clutch housing area that looks like wool or something it needs to be replaced. If you see large metal chunks it needs to be replaced.
Sorry i'm just remembering all the different things i've seen fall out of the clutch area after i removed a customers trans. Also check for hot spots on the pressure plate. That first pic has quite a few of them. They are the circle things. You can get the PP resurfaced, however i usually opt for replacing it. Flywheel should be resurfaced whenever you change the clutch as well. There should be no hot spots on it at all.

i wish i had pics of my stock one. I broke the pressure plate in 3 spots. Guys at Area 51 kept it for their collection. Guess it must not happen much.
looks like someone needed a new flywheel (big)!
I wish I had pictures of the clutch that went on my '96 RS. The clutch plate had shattered into pieces (went at 85mph..rev-matched to 4th and had to start out at two traffic lights with no clutch).
