Blown Headgasket?

So after a few trips to and from Alabama, I realized I have an oil/coolant leak somewhere on my motor. The motor is an lsvtec and I have already checked the vtec solenoid along with the distributor. I have traced the leak to the area between cylinder number 4 and cylinder number 3.
It appears that oil/coolant is leaking from the headgasket between these 2 cylinders.
Now for the questions..
The motor has 13k miles on it, it is possible for the gasket to give up already?
Could the head be warped even if I have never overheated?
Could the fact that the head has not been retorqued have anything to do with the leaking?

a little more info would be helpful. boosted/na, are they mixing in the pan? etc. do a leak down test, and take off the radiator cap, if you get air bubbles in the rad, while the cyl is under pressure, its either a headgasket or a cracked sleeve
