Can be a bad motor> blow by bad compression, or poorly adjusted valves. Also a problematic fuel system.

Could be the secondary 02 sensor or the catalytic converter...

check your EGR ... my was runing lean because the EGR valve was stuck... the mixture was not diluted by the exhaust gas throught the EGR like it was supposed to be...
see if that makes a difference.

The secondary is just used to monitor catalyst efficency. The primary is the only thing that would affect mixture. Unless your saying the cat isnt cleaning up the HC,CO and NOx.

Yeah, I'm expert on OBD2, but wouldn't a bad cat or secondary O2 be able to cause a code like that?

so you're obd2? plenty of things can cause rich. whats the fuel trim? probably negative...run a scan with a good scan tool. check all things that adjust fuel. check your FPR. check the MAF values.

well a bad cat may show more hydrocarbon output, but the actual mixture would be unaffected. A bad secondary wouldnt affect mixture, it would only throw code i believe, correct me if im wrong though.

you're right, the secondary just checks to make sure you're not polluting.
