Default jumpy tach..what to do? also, recommended auto trans additives?

hi everyone, my wife's 96 v6 accord (215k miles) has a really jumpy tach, i'm wondering if this is a common problem and is there an easy fix? it's always done this since we've had it, it does it only while driving - and i know it's not the motor racing.
also, i was thinking of using an additive in the auto trans maybe to extend it's life, as the car has so many miles on it. any thoughts on this? in fact, any suggestions you guys might have on increasing the car's longevity in general would be greatly appreciated. i've always had the motor oil/tranny oil changed regularily but am open to all suggestions. thanks a lot.
