Acceleration slipping

Default Acceleration slipping

Whenever i try to accelerate pretty quicky, it seems like something is slipping. Lets say i'm in 5th gear going 50 mph and i floor the gas pedal. The rpm shoots up and then later catches into gear. It doesn't slip in first gear though. Im thinking its my clutch but i have no trouble getting into gear. Or can it be the flywheel? Any help will be great. Thanks

I'm gonna guess your clutch disk is toast
