I have had my d16z6 swap for a couple months and ~2000 or so miles. Last week I was driving it and the car started shaking violentl for a little bit and then the battery light went off. I pulled over and saw the crank pulley hanging on for dear life and the accessory alternator belt missing. I towed it home and got a new belt and crank pulley key piece and then I drove it for a ~5-10 miles and then the same thing happened again. wtf?
try getting a new crank pulley..most likely the pulley is worn down inside, making it too big.
hopefully the crank snout is ok.
LOCTITE the crap out of the new one..put it everywhere, IMO..that stuff works wonders..let it dry too. might make it work. it'll be tougher to get off next time, but it might save ya

If you're gonna use Loctite, try this stuff (Loctite Sleeve Retainer):

It's more appropriate for the application, and let me tell you, this **** is hardcore for this sort of thing.

A friend of mine had the same problem with his B16. The previous shop did'nt tighten it all the way. The slot where the key went was twice as wide as it should have ben due to it just sitting there and moving. No matter what we did, the key would just "float" around in there instead of "sitting still". He sold the car before we could fix the issue.
