Default Wiring for Fan Relay

So along time ago when I did my swap, and didnt have a clue about wiring, I fudged the wiring for my fan relay. I have since wired the fan directly to a 12v source. I would like the fan to run off the relay though. Can somebody please lay out some detailed instructions on how to wire it? My motor is currently out of the car, so I can't go digging around with a volt meter off the fan relay. There seem to be blue/red and green/yellow wires that may have something to do with the system. 2 questions I need answered then:
1. What wires should I concern my self that enter the relay, and where do they come from?
2. What wires coming out of the relay should I connect to the radiator fan?

Please include wire colors. Thanks guys.

so what happened to the stock plug? its completly ripped out?

run a 12volt+ direct to the fan and a ground to a relay and and use your stock fan switch for the switch for the relay the fan switch on obd0 is on the back of the block and on obd1 on the thermastat houseing

is the fan relay switch a one or two wire plug? Im not sure what happend to the orginal plug- I did the wiring a long time ago. I think Im running the fan off of the old AC fan plug. I removed the entire A/C system, so the A/C fan is no longer installed. So, let me get this straight- hook up 12V to the fan, and run a ground wire from the thermostat to the relay, and then to the fan. how should i hook up the ground wire to the relay; what wire should I attach it to?

a relay has 4 pins 2 are mains and 2 are to switch the ground on put power to one of the switch sides and the ground to one of the pins on the fan switch and the other pin on the fan switch to a ground. the fan switch is green 2 pin on the back of the block. i would use a key on power source for the relay 12v+
