Default QTP E-Cutout

Is anyone on here running one, im having nothing but problems with it, either the allen set screws are falling out of the butterfly, it sticks open/closed, falling off with lock tighted bolts(high grade too), now the motor fell off the flange and dragged on the ground until it ripped off, luckily i heard it bounce off the bottom of my car and I found it. Anyone else have troubles with theirs?

I have it and I had all the bolts tack welded so they dont loosen up and Qtp customer service is very good.I asked them how much it would be for a new butterfly and bolts and they just said cover the shipping and sent me 2 sets

Im about to order one of these cutouts for my boosted 94 teg and I was hoping to get some feedback on them. One thing I have noticed is that the motor looks to be pretty big and I was wondering if anyone has had fitment issues when installing on hondas. Any feedback would be appreciated.
Thanks
