how to what? just pick it up at the dealer and remove the antenna with wire and then install it. but i would leave some type of pull strin in there just in case you decide you want the radio back.

 

Remove the two screws. Yank the antenna out. Screw in the new plate.

They sell block off plates on eBay for like $10-15 too.
