Over Revved, rattling sound, how to prevent?

Default Over Revved, rattling sound, how to prevent?

Alright, recently I was driving my d16z6 and floored it in first. It started to lose traction as the pavement wasn't as smooth and my motor is turboed at 8psi, the RPMs started to jump so I shifted to 2nd BUT it got stuck in transition from 1st to 2nd. And you know when you're in the spur of the moment and shift from 1st to 2nd, it's automatic to pound down on the gas again, however, this time it wasn't in gear (stuck remember? ) and it revved to 7800k on the tachometer.

Now, when I drive and the car hits higher RPMs, whether I gas it to like 5500k or I'm downshifting before a turn and the RPMs jump to about 4000k, I hear a rattling-ish noise from I don't know where exactly, but I'm guessing somewhere in the motor or near the exhaust? I have no idea. I would love to just have it be a loose exhaust but my guesses are worse.

So now I want to know what do accidentally revving over stock Redline damage? That way, I can upgrade it on my spare d16z6 motor so if something like that happens again, it'll handle it better. P.S. I'm sure we've all over revved before.. Some help out please!

So my questions are:
What could be that rattle?
What to upgrade to avoid something breaking if I accidentally over rev past redline?
And a misc. question: Does a rev limiter only set in when the car is in gear or should it set in when the car is in neutral?
