Loose crank pulley

I have 91 accord and few days ago i turned my car on and a loud noise came from the engine. I turned the car off right away. Checked the oil level and everything was fine.
After further troubleshooting, i noticed that the crank pulley is wobbling and it’s hitting the timing belt cover. I parked the car for the week and and took it to the shop where i work.
Once i took the pulley off, it wasn’t a pretty scene. The locking pin was damaged and crank pulley was no good. Long story short i ended up replacing the locking pin which was a b**** to take out and the crank pulley.
Now to my question, the only way for this to happen is if the crank pulley wasn’t torqued down and this is the second time i had the crank pulley changed. Has anyone experienced any problems with their crank pulleys?

most likely it was not torqued down properly, chances are the threads inside the crank pulley are damaged at this point so check the crank pulley bolt once a month or so just to make sure its tight. i have had 3-4 pulleys on my wife's prelude for the same reason. previous owner did not torque it down well, it backed out and destroyed the key and pulley, new key and pulley, happens 3 months later, changed it again, and this time put green loctite on it and it lasted nearly a year, i jsut changed it again last week.
my plan is to put my h22 into her lude so i cna pull her engine and repair the crank.

yeah, it sounds like the crank pulley bolt is just loosening up, like above, try some locktite on it if the threads are not damaged.
ive actually had one completely fall off while i was driving, it wasnt good, you dont want that to happen...

Funny to see this thread. We had a freakin plauge of cars with loose crank pulleys come in (civics, preludes, and a few accords). You can hit those things with an impact for 20 minutes and it could still back out again. We found the key is a medium strength thread locker like a few others have said. But without sometimes torque doesn't seem to matter (in those rare cases).
Locktite Red comes to mind. But yes, it's important that you get a handle on this. The Indy Honda garage I associate with has a crank snout on their write-up counter which broke off from this. He uses it as a pencil holder.

yeah, i use couple drops of oil instead, it's not so much that it need some extra grip, what happen most of the time when torquing it down it, the bolt is dry as fwak and the torque wrench is reading friction torque instead of real torque number
