are there any restrictions? if so, how many and what do they neck down to?

The narrowest diameter on the whole b-pipe is the area near the flanges, and the ID at those points are still 2.5" ID.
The sound of the Kakimoto R is 96db and under with deep growl. So pretty quiet, but people will know you have an aftermarket exhaust!!

if the area around the flanges is 2.5" and is the narrowest what is the diameter at the widest and through the WHOLE b-pipe??

The muffler is straight out, the big muffler and the two resonators are there to reduce noise!!

The last time I got to see this exhaust in person, it does seem like the exhaust is not free flowing, but it is quite free flowing. The big cannister in the back (muffler) acts as a sound dampening muffler (big one too) It is a straight through design and quiet muffler for those who want a relatively free flowing exhaust without the fart sound coming out from other N1 exhaust systems.
Yes there is a bottle neck at the cat flange... it is 63mm. But that is neccessary if not there is no way that there will be flange that can accomodate a 70mm piping while bolting up to the cat flange!
Take a pic of the flange that bolts up to the catalytic converter. It looks like there is a bottle neck there.
As soon as this freaking winter is over (hopefully in a month) I will install the exhaust on my ITR. I will take some pictures and post them. The Kakimoto is very high quality and is very free flowing. I will have the car on the dyno this spring but unfortunatly it will not show the increase of only the exhaust as I will also tune the Toda spec B.
