Default '91 Accord Tranny slip?

Hey guys, Im mostly in the integra section, but i do also have a 91 accord . It's seen its day with 480,000Km, still running strong but the tranny has been getting worse and worse. Today as I accelerated in 1&2 etc and once i hit about 3000rpm it seems to slip and cut out causing the car to sputter, its Auto btw.
What could this be, just an old tranny dying, anyone know specific problems? It has always shifted rough and sticks around in gears for a while before slamming into the next.
The fluid hasnt been changed for some time but I did top it up today, it was half empty.
Also, i searched but didnt find anything specific on this.
Thanks for any info

I'd say the first thing to check would be the trans pressure cable that comes from the throttle.
