Default 95 civic dx ecu ??

is the ecu in a 95 civic dx non-vtec already set up to accept a vtec d15, or does it need to be chipped/reflashed? also is the main wiring harness pre-wired for vtec or do I need to swap that too?

No, it should have a 02D011F0-1500 board, so it is easier to convert.

No, you need to add vtec to the engine and chassis harness or buy a EX/SI harness and just add vtec to the chassis harness.
