ok wait.. what part fell out? the input spline to the tranny? or the hub of the wheel?
do you have a swap? is your car lowered?
if you have a swap, are you using 92+ integra axles?
and it could be as simple as the retaining ring inside the diff is worn out, or missing.

Axles don't have cotter pins in them...do you mean the retaining ring clip that holds the axle in the tranny? The only cotter pins that could effect the axle are the ones that retain the castle/crenelated nut on the ball joints.

yes.. the axle has expanding retaining rings. if its not pressent, the axle can walk out of the differencial. thats what you need to check.
