Default need help. confirm this. putting motor back together, when start timing belt slips

ok so im putting my motor back together and i go and start the car after setting everything to tdc. once i try to start i can hear the car turn over but then i hear a slight noise of pressure letting loose. i go out and check and see that my cranks tdc is no longer in line with my cam gears tdc. im sure im doing everything right.

i did a search and came up with replacing my tensioner. can someone confirm this. and does this happen often?

when putting belt back. here's how i did it.

set crank pulley to tdc mark. set cams/gears to tdc by setting them to 0 and also looking into the cam caps check. now pull intake side of belt up and over the cam gears then slide other side on. should have 1 in deflection in between gears. turn crank pulley over 3 teethes then tighten tensioner.
