Can someone tell me all the possible reasons for reving idle?

Can someone tell me all the possible reasons for reving idle?
changed IACV. No change. 
Does CEL cause vtec not to work? My engine is redlining at 5500 rpm (f22b1 in 90 accord). I have no vacuum leaks. Would a reduced diameter on fuel line (I used connectors with hose clamps) cause this (reduced 5-10%). I also ran wires for the new map sensor, Do I need to disconnect the old one on the firewall? Can that cause idle/code?
