Default Problem when applying brakes

I own a '92 Honda Accord and when I apply the brakes at 50 MPH and up I get a shimmying in the wheels that causes the steering wheel to shake as well. I installed 4 new brake pads around 1 1/2 years ago. Could it be that my rotors are uneven? Any clues???

it could also be that the lug nuts on your tires are too tight...i really don't know why this happens but it does...
