CEL code 14, stumped me.. can't figure out how to fix it.. HELP!! *long*
What I did:
About 3 days ago, i changed my auto intake manifold out for a skunk2 intake manifold. My car was an auto before and i converted it to a manual thats why i was running the auto intake manifold. When i changed dropped in the skunk2 im, i also put on a h23 throttle body. i unpinned the orange line which was A13 and moved the pin at A14 to A12. The 3 wire iacv plug was cut off, orange wire left hanging, spliced yellow/black from car to yellow/black on new 2 wire iacv plug. And the black wire to the other wire of the 2 wire iacv plug. The throttle body was an H23 throttle body, it has 2 tubes on top of it. The small straight one is for the map sensor, so i used a firewall mounted map sensor off of a 5th gen civic. And the other tube is a larger upward curved tube. This is for the little charcoal canister, but mines already hooked up so i left this one open(unhooked). Now i have the two old iacv coolant lines left, so i routed one of the old iacv coolant tube to one plug of the fitv on the h23 throttle body, the other side of the fitv goes to one plug of the new iacv, and the other plug of the new iacv goes to the second old iacv coolant line. Now everything is hooked up. I refilled the coolant and bled it. Next reset the ecu.
What happened and my attempt to fix:
I drove the car and it was fine for about 30-40 minutes then all of a sudden, everytime i rev to 1500-2000 rpm the car would jerk like crazy and the CEL came on. I pulled over and checked the code and got a code 14. Its dark now, so the next morning i tried to fix it. First i checked to see if there was voltage coming into the yellow/black line at the iacv plug. I got 14 volts so its cool. Then i checked for voltage at pin A12 at the ecu and got 10 Volts there. So looks like my wiring it good. I then checked to see if the iacv was clogged and clicking. Wasn't clogged cause i cleaned it before i put the skunk2 im on. And it did click. But i had another iacv laying around and i sounded like it clicked louder, so i cleaned up the spare and through it on. I resetted the ecu, and drove it for a bit. Seemed fine, so i parked it. Next day i drove the car to work and back and it was fine. Day after, i was driving the car around maybe for about 30 minutes and the problem started again. The car would jerk around when i rev to 1500-2000 rpm or so, and also bog a little at about 2500-3000 rpm. The CEL came on, and got a code 14 AGAIN
I don't know what to check out or change right now. Some stuff i was thinking about:
the fitv, my auto manifold didnt have one. should i plug this up? could this cause the problem?
the nipple/tube on top of the throttle body, am i suppose to plug this in somewhere or block it off?
did i route the coolant lines right?
what else could trigger the code 14 and cause this sort of problem?

just remembered that i forgot to mention some stuff.
this problem is on my '97 civic ex with a d16y8, and the manual p2p ecu i used, when i plugged it in, i was only able to plug in 3 plugs, 4th plug wouldn't fit. i'm guessing this is the auto tranny plug?

check your wiring again. either the ecu didn't got the signal from the sensor? be sure that you are using the right ecu.
