Default Valves adjustment problems pls help..

When I adjust my valve clearance from cam lobe to the rocker arm, all of my 15 valves were perfect measurement, but at the last valve (left side #4 exhaust) touching the lobe. I cant even fit any side of my feeler gauge. I unscrew the rocker arm all the way and it still hitting the cam lobe making the valve open...

Could someone could help me what wrong.. I was thinking of the valve problem, but all 16 valve are in the same box and spring also.. The cam shaft is in perfect condition...

Please help..

place each cylinder at TDC and adjust the valves to spec...You will only be able to do one cylinder at a time and then youll have to rotate the engine via crank pulley to set the next cylinder at TDC.The lobes should be pointed kinda horizontally and off the rocker arms when its at TDC...
