Default Possible Clutch Problems?

I have a 01 Accord Ex 4 cylinder Manual with 90k on the engine and clutch. I have noticed that sometimes (very random and cold or warm engine) I will have problems getting the car into 1st gear. Of course this only happens when I come to a complete stop (at lights and maybe stop signs ).

I would fully depress the clutch, and put the car in 1st (where the stick would stay) only to not engage when I let off the clutch and press on the gas (it would grind at this point and go to neutral). I would then re-press the clutch, throw it in second and then back into 1st and everything is fine and engage. It would occur very randomly but not very often (1 in 20 stops).

Should I bleed my clutch? Adjust the clutch pedal? Is this a sign of something more serious?
