Help with a hesitation problem

Hopefully it's ok to ask a generic problem here
I have a Honda Accord (Euro) CU2 model with a K24Z3 engine which has developed a hesitation problem
It happens only rarely, but sometimes when I stop at traffic lights, and then pull away, after pressing the throttle lightly, the car appears to be responding to the throttle but the engine doesn't give me any power at all. If I then punch the throttle all the way down, after a second and a splutter, the car takes off at full speed! No check engine light is lit. I've also noticed that sometimes, there is a rough idle, and the problem appears to happen more often when stopped going up a hill (hill start).
A few months back I swapped in an RBC intake manifold. Everything was OK for a few months but hten this problem developed - so it's possible I did something wrong, so bear that in mind, but I would've expected to have a problem immediately after fitting not months later
The car also has a catless downpipe fitted, and a high flow cat, although this was on the car for 1 year previously without problems.
Here is my list of what I think could be possible causes so far:
Injectors - when I fitted the RBC I used injector adaptors, but fitted new O-rings, and gave the injector body a quick clean, so I don't believe a problem is there. I have some refurbished RDX injectors on the way anyway, so i'll probably change them, and set the injector size in Hondata, so I can rule those out I suppose.
Throttle body - I've removed it and cleaned it with carb cleaner. There was some build-up but not a lot. I've run the recalibration/test function within Honda HDS and this tells me nothing is wrong. It's very expensive to buy a new one.
Vacuum leak - I've checked all hoses for fitment and I can't see anything. I need to test it with propane/brake cleaner though.
Spark plugs - Were changed with Denso units during a service 1 year ago so they should be fine
I don't see any throttle position sensor / ("APP sensor") that can be replaced separately looking at the parts diagrams
I've run some fuel system cleaner through the car on the last 2 tanks too to no avail.
The car is running a stock map at the moment, I don't know if that would cause it? I will get it mapped eventually on Hondata Flashpro but waiting for my exhaust to arrive.
Has anyone had this problem before - any ideas?
