Replace this distributor rotor and cap ?
It is original from 1997 with 190 000km (120 000 miles). It's working fine, I'm just curious if it is worn enough to replace it? Can someone say from the pics that it's worn or people usually replace it preventively? What can happen if I will not replace it and it will fail somehow?

120k miles?
YES! Replace it!
You will get better gas mileage, smoother idle, less chance of a high rpm misfire etc etc etc.
Plus it should only cost about 30 bucks and take less than 1/2 hour to do.
If your spark plugs and wires are original as well, might as well change them too. Its called a tune-up.

As a temporary fix you can sand the distributor rotor edge and contact areas in the cap.
I had a dizzy failure some time ago- my rotor went into pieces at 140km/h.... Baaaad- but it was an Italian replacement crap. But it scared me a lot because motor was making an awful noise.

No. You don't want to use cheap parts. Sure, they'll work for a second - but they will eventually cause more problems.
If it only costs $20 for quality parts, don't you think the dealer would use them? Of course they would. But there's a difference between cheap and quality.

Point being? You guys can do what you want - but there's a reason why dealerships don't use those cheap parts. I guess you could buy them for $20 - but you'll be buying them very often. that $20 will muyltiply exponentially - then you'll wish you'd purchased a quality product.
I would never go cheap on anything electrical. Just not my game. Oh, if you can find them for $20 - go ahead and buy two for that day you're stranded on the highway.

WTF dont waste ur money replacing that, that cap looks brand new, usually the rotor gets so worn that it breaks and that middle metal piece in the cap will get so old and corroded that it actually disengrates. WOW i cant believe you ppl think youll get better gas mileage. That cap is in great condition, and if it does fail your car just wont start....put it this way my prelude has 201,000 miles original cap, go figure
i could care less if your distributor cap and rotor lasts a billion miles, its called preventative maintenance and it WILL improve performance and fuel economy. Do you look at engine oil and say "this stuff looks fine, it can go for another 10k miles easy!" No, you change it at regular intervals.

You can either do it now while you have the free time, its a nice sunny day, you have all your tools etc. . . .or you can wait for it to fail and leave you on the side of the road in a thunderstorm with trucks flying by inches from your head while your girlfriend is bitching about being late to the most important job interview of her life, trying to remove a distributor cap with a pocketknife.

blake i gotta agree with the weatherman on this one, if you get a shitty cap or rotor, it could fry your coil or ICM, and then your up the creek for more than honda parts would have cost in the first place
believe me, even with honda parts a new rotor fried my internal coil, theres sensitive equipment in there.....
OP: yes that needs replacing, but no it wont cause problems how it is...

eh, i've never had problems with the caps and rotors from O'Reilys. I've inspected a honda caps and rotors and the replacements and they appear to have the same quality.
oh well, thats besides the point of this thread. Point being, it needs to be replaced, regardless of which one you decide to replace it with.

oh i can almost garauntee you that theyre 100% the same, 99% of that crap all comes from the same place...i just dont want to catch that 1% chance, getting my hands on another distributor wasn't fun....you would be surprised how many products are sold by different vendors but are the exact same....cough valvetrain cough.....

ok if youve actually compared that cap and rotor to one that doesnt work at all, the one pictured looks almost brand new, that center *** will actually be disintergrated, w/e not my money im wasting
I have gone about 40k with a cap, rotor and wires from o'reillys and haven't had a single problem so far.
I have also sold many cap and rotors for hondas at o'reillys and have not had a single complaint in my two years working there. If it does fail, take it back the Borg Warner brand carries a lifetime warranty

if you haven't done so in a while - change your distributor cap, rotor button, plug wires, and plugs
