Need help diagnosing a problem

Hello everyone, just made an account here after being redirected by reddit mods on r/honda after trying to post this there.
So, my mother drives a 2005 CRV, 4wd, with the 2.0 petrol engine and called me about a problem she's having on the car. She told me the whole car shakes / vibrates / resonates very violently (around 335 beats per minute) when the car is in the highest gear (4th) at around 1750 rpm when driving on a slight uphill so the engine is under some load. So I went to visit her and went for a drive with her. And lo and behold, at a very slight uphill, in 4th gear around 1750 rpm the whole car tries to dismantle itself (again around 335 beats per minute). I told my mother to take the car to the local repair / maintenance shop but she said the car had already been to a maintenance shop for a week only to have the guys say they have no idea what it is, sorry, drive safe. I'm somewhat confident the problem is something engine related since once the vibration / resonation starts it goes away immediately if you release the throttle for even one millisecond after which you can come back on the throttle as nothing happened. She told me this has been happening for about 6 months now.
The car hasn't unfortunately received regular maintenance but from what she could tell me was:
Oil, filter, sparks, intake filter and cabin air filter were changed 4000km ago. (The car has 320 000km in total)
Also the knock sensor broke around Christmas 2017 and was replaced at the very place she was told they could do nothing.
Could this just be them scamming her for the knock sensor (god I hope so because that would be an easy fix), or could this be something serious to the point she should be looking for a new (used) car.

You didn't say if its an automatic or manual, however i suspect auto as you said 4th gear is the highest (although an 05 may have a 5 speed auto, but in your market the four speed is probable)
So with the Automatic, check and likley replace the trans fluid.
In fourth gear at the torque converter clutch (TCC) will slip and chatter if the fluid is contaminated...this can occur at light to moderate loads when the TCC applies. It will do a partial and then full apply and contaminated fluid will make it slip. The test for this is push the D3 button when the event occurs as this will release the TCC. If the issue subsides, that's the culprit.
The fluid should be replaced a minimum of 3 times ( drain and fill, 3 liters each time) with a drive in between. Typically I change it once, and the condition clears up. But if I suspect heavy contaminant ( clutch material in oil) i will change it 2 times after or more. Also, there is a replaceable in line filter just under the radiator about center. To access, remove the plastic cladding underneath.
So with the Automatic, check and likley replace the trans fluid.
