Default 94 Accord (V-Tec) Idle drops too low while in D or R

Hello to all. I am desperately looking for some guidance about this problem.
My sons 1994 Accord (V-Tec) 4 door idles fine even when cold and warm.
Now, here is where it gets bad. When the car is idling, and we go from p to d, it will idle fine in D. It is when we go from P to R, things go haywire. The car lunges forward a bit after the initial gear engagement and then the idle will drop to about 200 r's then die.
It only does this when it is warm. The car idles FINE when not in gear. But to re mention, it is not untill we go from D to R or R to D.
The lock up solenoid tested good. I have not checked the shift solenoid as of yet. Will do in a few minutes.
When I pulled the codes, The D indicator gave me: "18-13 and 14" codes.

hmm. im not sure bout the codes, when was the last time it was tuned up? I mean full tune up with throttle body cleaning and new filters.

that's probably the first thing I would address if its been forever and a day since the last time it was done... got a hames book? its got the codes... or heck just use the search.... anyway figure out what those codes are and go from there, bout all I can tell ya.

i have the service manual.
Update.
I adjusted the idle via air bleed screw and disconnecting the IACV. Shut off car. Pulled 7.5 amp fuse.
Sat in the driveway going from R to D to P. D to R to P. Over and over again. Car IDLED PERFECTLY!
Drove it around the corner. Came back with the Going From P to D, now the car goes into D, Then you notice a Lunge forward and then the car dies. It is like the Transmission converter is locking up. As the Tranny is placed from Either R or D to Park, I hear a very noticeable "Rattle" coming from one of the shafts or bearings inside the tranny. I checked lock up solenoids ohms and functionality. They checked good.
Now at idle the car is hunting for the proper idle. Finds it and then starts hunting "surging again". I think it may be in the tranny or the TCM. Any clues if I am on the right path?
Thank you for your reply. Car has just had a complete head rebuilt. 4 valves on cyl 2 were bent.
Plugs are new, I cleaned the TB when I got the car back from the shop. Do you know if I
can check the ECM and TCM with out the "dealer suggested" Adapter between the units?

Also, if the gear selector is placed in L or 1st, the car dies instantly.
And the symptoms happen more frequently and more severe.
Could it be an transmission internal problem?

hmm. that is terribly vexing. what does the trans fluid smell/feel/look like? check it directly after cycling the shifter as you did previously. if its physical damage it could show some clue in the fluid... or lack there of.... a 60k service on trans fluid takes a couple of min and only a little over two quarts of fluid. im not a real whiz when it comes to computers and electronics but it seems as though your problem may be mechanical.... my specialty.
