Default Wastegate sizing question

When a manufacturer refers to a 35mm, 38mm, 40mm, etc wastegaste, what does that specifically refer to?

The underlying question here is are all wastegate flanges on turbo manifolds the same size?

For the most part manifolds come standard with the 35/38mm flange. And usually going bigger is a special request.
