when it doesnt do this, what is your resting idle?

Have you messed with the throttle stop screw at all? Or adjusted the throttle cable at all?

Is your engine/head OEM rebuild?

What is your fuel pressure set at? Do you have a gauge on it? Check to see if its constant with pressure or fluctuating/losing pressure. Fuel Pump could be slowly showing signs of it being on its way out??

If all of those things are OK or new or checked, if you have another MAP sensor laying around try that. Also reset your ECU when installing the new MAP sensor.

With that much work done it really needs a proper tune. Something just the slightest bit off could be causing a intermittent start issue.
