Whats up guys?

I need some help figuring out whats up with my car (95 integra ls)...My temp goes straight to hot after like 5 mins of driving, the tube that feeds from the radiator to the reservoir puts out a good amount of steam, and finally it continuously revs from 900 to about 2k whenever it overheats but calms down eventually..

Any help is appreciated..
