Isn't a B16A a vtec motor? Why would you run a stock LS ECU on a VTEC motor?

Black smoke is caused by excess fuel that has entered the cylinder area and cannot be burned completely. This could be caused by a faulty ecu (ruled that out though), leaky injector, or a faulty fuel pump. Check your oil and make sure that the fuel has not contaminated it.

without your o2 sensor you should be pulling a CEL, when you dont have a o2 sensor the ECU goes into closed loop and runs pre-programmed fuel maps that honda included in your ECU which may very well be SHOT.

It's probably that, how is your engine supposed to know a correct air/fuel mixture without it?

Since you have a 90 I'll assume you have OBD-0 or 1, so you need one 02 sensor. As far as the diagram, SEARCH, SEARCH, SEARCH.

when he doesn't have an o2 plugged in, doesn't it mean that the ECU is dumping the maximum amount of fuel into combustion as if it were WOT or during a cold start? that would explain the bad gas mileage...

This is correct.

Obd 0 is 2 1 wire O2 sensors and they are different. They read different voltages to send to ecu so they can not be "Tapped" together.

OBD1 has 1 wire, 4 wire and 5 wire O2 sensors depending on the engine.

Maybe not so much 1 wires but cx, dx, lx, ex, and si have 4 wire and VX have 5 wire.

Hope this helps and yes without an O2 you will run pig rich. But you should be throwing a code. Check the light on your ecu I bet that its blinking.

Are you running an aftermarket header? You might just have to get it tapped for an 02 bung.

P.S. I beleive it's pre-cat
