Do the basics first, like check fuel pressure. You said you changed the pump and filter, but why? Were you hoping to get lucky or did you confirm low fuel pressure?

Sounds like input shaft bearing. Very common. If thats the only thing internally wrong, just do the bearing and seal and call it a day. You can save a lot of $$. Ive done a handful of input shaft...

Door caps? You talking about the plastic covers near the B pillar? If so, I think I used SEM trim paint. My car is garage kept and they're still holding up nicely.

Looks like it to me too. Notice the carbon buildup isn't as bad for that cylinder. Burning coolant will actually clean up some of the carbon in the chamber. Id have the head checked at a machine shop.
