Default coolant leak through the upper radiator hose

recently i'm experiencing a coolant leak on my 92 honda civic with d16z6 swap. the leak is coming from the upper radiator hose. the hose was replaced and i even push it in some more hoping it will stop the leak. i even tried bleeding the radiator by taking off the cap and let the car idle. coolant will fill up to the top, but once the fan turns on, it will go down. whenever i drive, it will continue leak. anyone have any suggestions? could this be the radiator problem?

maybe the clamp holding your hose is not that tight. could be the wrong cap maybe, causing too much pressure to build up.

Hmm, the clamp should be good since I bought it new. Radiator cap might be bad since I don't even know how old it is. Could there be any possibilities? Maybe a bad radiator?
