Default 1999 Honda Accord 4cyl with idle problem

Hi there, I have a 99 accord, 215 k on it, it has a rough kind of vibration at idle when warm, idle still seems to stay at 750 rpm, but it seems to vibrate and you can feel it in the steering wheel pretty bad.Also noticed that over the stereo it kinda distorts the music also, once tho you step on the gas, things are fine.i have had the timing belt done, not too long ago,same with the valve adjust, Ngk plugs only have about 25,000 k on them, this is something that has come on in this car within the past few months now. Last night i pulled the plugs just to look see, they seem okay, I checked the cap and rotor, look okay, and using the big thick Honda accord s/m i bought, i checked the 4 plug wires resistance like they described,all check okay.Checked the timing with a light,incase something went wrong there, its dead on.I also checked the coil out too, I get no reading,even on the lowest setting on the ohmmeter from the A to B terminals (primary side) but checked the secondary side and all is in spec. Am i still doing this wrong? It seems straight forward in the service manual.car runs fine otherwise,with no issues otherwise.Am I missing out something to look at.Its driving me nuts!! i wonder if its electrical at all?? thanks for your help!!

This is in response to my previous post,about the same issue..The idle seems to make the car shake,yet its not the typical say the idle drops way down to like 500 rpm it still seems like the tach is close to 750 rpm,with or without loads on the engine,the idle stays much the same yet the shake is there,idle,or in gear.
-I've done the t-belts in the car,valve adjust also at 190,000 kms,car is now at 225,000

-Plugs,cap and rotor still seem to be fine,although they will be due for replacement at the next '48' service.plugs show no signs of fouling.. lean/rich/antifreeze

-Spark plug wires are resistance checked okay,I have checked the intake air temp sensor,and removed and cleaned the throttle body ports and the rotary idle air control valve

-I have no codes,no misfires,no egr codes,no vacuum leaks that i can find,and pcv valve is new.I looked at the balancer pulley,doesnt look like its coming apart.

-Engine mounts all okay,re torqued them and had replaced the rear one because i thought it may have been the culprit

-Is it possible that the balancer shaft belt had jumped?there is a bit of piston slap that i can hear when its running,I believe its at #4 cyl,would a weak cylinder cause this,or am i missing something like a poor ign.coil? I should also note in out colder climate,it starts acting up and vibrating from -15 and colder...

-Id hate to try and compensate by twisting up the idle stop screw under the throttle body,I know its not a new car anymore but i like it and just cant figure this out still,maybe i still missed something can anyone shed more light on what i've done so far? thanks muchly! Kevin..

off on left field, but some people on this site chalk it up to clogged EGR ports from too much carbon build-up

so, add this to your list of things to check
