DC Clutch / Tranny question

DC Clutch / Tranny Problem HELP!
Let me start off by saying ive got a 94 dc with a LS motor and GSR Tranny.
Clutch died 2 months (Springs were popping out the disc) installed brand new flywheel and clutch for XTD stage 2 (Press plate, Kevlar disc, 10 lb flywheel, Throw out bearing, Pilot bearing, New Master and clutch slave.)
Now my question is will a bad input shaft bearing wear out a clutch faster? My car makes a nasty chattering noise when the clutch pedal is let out and the car is in neutral also makes kind of a grinding noise when I rev the car up (did this before i changed the clutch i thought it was a bad Throw out bearing). The chattering noise and and grinding noise are completely gone when i push the clutch pedal just a little bit in. The clutch is starting to slip a little say for instance im in 5th gear doing 20 and mash the gas it'll slip then catch and start acceling. (My car did not slip when I put the new clutch in and it was broken in properly).
Other known problems.
slight rattle/vibration in the shifter. (Brand new Energy motor mounts both front torque mounts)
when i push my clutch pedal to the floor the car wont go in gear rolls forward when i try, grinds syncro's when shifting gears
^ that problem dosnt happen when i push the clutch pedal halfway.
Any help would be much appreciated.
also does anyone know the part number for the input shaft bearing?

PLEASE HELP
Maybe i need to adjust my clutch pedal? when its in first gear and i let the pedal out it engages really late. (But this dosnt fix the grinding noise coming from the tranny and the chattering.
