5th gen 5 lug conversion?????????!!!!!!!
1st gen Odyssey parts, I believe. The only issue is that the Odyssey's parking brake is used on the front calipers and not the rear like ours, thus you'll have to find a way to hook up your parking brake line to the rear calipers.
u serious?? all u need is 1st gen odyssey parts and thats it??? bolt right into the 5th gens??
AFAIK you need the wheel bearing, hug, rotor, caliper, and pads.

Has anyone here actually done it?
Yes, it's the 1st gen (95-97) Odyssey parts.
The lug pattern is 114.3x5, the same as the 5th gen Prelude. My plans are to do the Odyssey hub/lug/brake conversion and use 16" Prelude rims with the same bolt pattern, thus keeping the car all Honda and making it more of a sleeper.
Misa suggests attempting to use the Accord rear caliper in order to keep the flange for the parking brake cable.

When I said you need the hub, bearing, caliper and rotor, I meant front AND rear from the 1st gen Odyssey.
However, it was suggested that the rear Accord caliper may work, so we can keep the flange for the parking brake cable.
Who runs 5-lug front 4-lug rear?

No, the Odyssey has rear disc, look at the pictures G-MO posted.
Also, why would you want to use brakes from a smaller, lighter car that doesn't need as much stopping power?
