"D4" on shifter flashes when in gear

"D4" on shifter flashes when in gear
The car is a 96 Accord LX V6 (automatic). When put into gear the light on the shifter "D4" continually flashes. Also, the car is shifting hard through gears. This is going on what he has told me. Any ideas?
He thinks it may be the TCM (tranny control module), but not sure.
